1. Accomplished Application Form (EMB EDA Form 002)

2. Affidavit of Compliance with the Minimum Wage and SSS Laws (Pro forma text attached, please transfer to your company?s letterhead). (EMB EDA Form 003)

3. Audited Financial Statements (Immediately preceding taxable calendar year).

4. Bank Certification of Export Performance same year with No. 3 (Inward Remittance of Export Sales).

5. Accomplished EMB Company Profile Form (EMB CPF Form 001)

For Indirect Exporters (additional)

Sales; Certification from the direct exporter attesting purchase of raw materials from indirect exporter and re-exportation of finished goods.

For construction services exporters (additional)

Proof of international contract

Note:

For CPRS purposes, Export Sales should be 50% of the Total Sales for the year.

For BIR purposes re: ZERO RATED VAT, Export Sales should be more than 70% of the Calendar Year Sales?
